Understanding the Success Rate of TESE

Posted by Nick Harper, reviewed by Dr. David James Simpson | 2024-Mar-07

As someone who has struggled with male infertility, I've spent a lot of time researching various treatment options. One procedure that's come up time and time again is Testicular Sperm Extraction, or TESE. But what exactly is the success rate of TESE, and how does it stack up against other infertility treatments? Let's take a closer look.

TESE, also known as Testicular Sperm Extraction, is a surgical procedure used to retrieve sperm directly from the testicles of men who have azoospermia, a condition characterized by a complete absence of sperm in the ejaculate. This can be caused by a variety of factors, including genetic disorders, hormonal imbalances, or blockages in the reproductive tract.

During a TESE procedure, a small sample of testicular tissue is removed through a small incision in the scrotum. This tissue is then examined under a microscope to locate any viable sperm, which can then be used for in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI). The goal is to bypass the blockage or lack of sperm production and give the couple the best chance of conceiving.

But how successful is TESE, really? According to studies, the success rate of TESE can vary quite a bit, depending on the underlying cause of the azoospermia. In general, the success rate for retrieving usable sperm during a TESE procedure ranges from 30% to 60%. This means that in 30-60% of cases, the surgeon is able to find and extract viable sperm from the testicular tissue.

However, it's important to note that this is just the first step. Even if sperm is successfully retrieved, the next hurdle is using that sperm to achieve a successful pregnancy through IVF or ICSI. Studies have shown that the pregnancy rate following TESE and IVF/ICSI is around 30-50%.

So in summary, the overall success rate of TESE – defined as the percentage of men who are able to father a child using sperm retrieved through the procedure – is typically in the range of 15-30%. While not as high as some other infertility treatments, this can still be a viable option for many couples struggling with male factor infertility.

Of course, these are just general statistics. The success of TESE can be influenced by a number of factors, including the skill and experience of the surgeon, the underlying cause of the azoospermia, the woman's fertility status, and the quality of the retrieved sperm.

I've heard personal stories on both ends of the spectrum. Some couples have had great success with TESE, while others have struggled. One friend of mine was able to conceive twins using sperm retrieved through TESE, while another couple went through the procedure multiple times without any usable sperm being found.

So what's the takeaway here? I think it's important to have realistic expectations going into a TESE procedure. It's not a guarantee of success, but for many men with azoospermia, it can be a valuable option worth exploring. The key is to work closely with a fertility specialist who can evaluate your specific situation and help you make the best decision.

Have you or someone you know gone through TESE? I'd be really interested to hear about your experience – the highs, the lows, and everything in between. Leave a comment below and let's continue this important conversation.
